Pennsylvania Statewide Adoption Network (SWAN)
Child to Family Connections is a proud affiliate of the Pennsylvania Statewide Adoption Network (SWAN). SWAN is a partnership among the Department of Human Services, the Pennsylvania Adoption Exchange, public and private adoption agencies, organizations, advocates, judges, the legal community, and foster and adoptive parents. The goal of SWAN is to build a collaborative process and to expedite permanency for all children and help prepare them for their forever family. Every child is eligible to receive SWAN services, as soon as they enter foster care, regardless of their permanency goal.
Child to Family Connections provides Child Profiles, Child Preparation Services, Family Profiles and Post Permanency Services to children and families in the northwest Pennsylvania area. Since 2006, Child to Family Connections has finalized over 40 SWAN adoptions. Child to Family Connections and its experienced staff prides themselves in their ability to help all children move towards permanency, as every child deserves a family to call their own!

SWAN family profiles are written for families who wish to adopt children from the child welfare system. The Family Profile preparation consists of a series of meetings and trainings in order to determine if your home can meet the state and agency requirements to raise and adoptive child. Meetings provide a chance to ask questions about adoption and to discuss what type of child that would fit best in your family. The completed profile is used to match families with waiting children. Attention Adoptive Families!!
Have you recently adopted a child or are providing permanency through Permanent Legal Custodianship or are a Kinship Care Provider?? Post Permanency services are available to support the child and family following adoption or legal permanency. All adoptive families are eligible for Post Permanency services, whether they adopted through a private or SWAN adoption. This unit of service is intended to preserve families through support and provide resources and connections necessary to ensure a healthy, lasting relationship. Case advocacy, support groups, and respite are three services provided through Post Permanency. Case Advocacy ensures that families receive access to services designed to meet their needs after they have made a permanent commitment to a child. Support groups help build relationships, form a community of mentoring, provide education and offers socialization and recreational activities for families and/or their children. Respite services provide families with planned breaks to support a consistent level of parenting.
