Adult Direct Support Services (ADSS)

 

Services are provided on a referral basis from Crawford County Human Services.

The Adult Direct Support Services Program’s goal is to enrich or add to each of our individual’s lives at home and in the community by helping them achieve independence.

Each individual’s personality, interests and skills are taken into consideration as their Individual Service Plan Support Team design activities that will enrich their life.

An individual must be eligible to receive a referral from Crawford or Venango County Human Services. Once the individual chooses to partner with CFC, a suitable, compatible Direct Support Professional begins supporting them to reach their goals and reach beyond their expectations.

 

 

Life Sharing

In Life Sharing, a family opens their home and heart to share their love, support, friendship and positive, supportive family life with a person who chooses them.

An individual must receive services through Crawford or Venango County Human Services to be eligible. Life Sharing homes adhere to 6500 regulations in a licensed environment.

Individuals are provided with opportunities for services may include: assistance with daily needs, training, support for the acquisition, maintenance or improvement of functional skills, personal needs, communication and personal adjustment. Individuals are encouraged to become contributing members of the Life Sharing home. Participation in community life, volunteer or civic-minded opportunities and memberships in organizations are encouraged.

Individual needs and preferences are considered when making the decision to become a Life Sharing home. The process of welcoming an individual with unique needs includes a transition period, where the family and the individual get to know each other. A placement will not be made unless it is a good fit for all parties involved. A stipend is provided to help offset the expense of basic house costs.
