ATTENTION ADOPTIVE FAMILIES!!

 

Have you recently adopted a child or are providing permanency through Permanent Legal Custodianship or are a Kinship Care Provider?? Post Permanency services are available to support the child and family following adoption or legal permanency. All adoptive families are eligible for Post Permanency services, whether they adopted through a private or SWAN adoption. This unit of service is intended to preserve families through support and provide resources and connections necessary to ensure a healthy, lasting relationship. Case advocacy, support groups, and respite are three services provided through Post Permanency. Case Advocacy ensures that families receive access to services designed to meet their needs after they have made a permanent commitment to a child. Support groups help build relationships, form a community of mentoring, provide education and offers socialization and recreational activities for families and/or their children. Respite services provide families with planned breaks to support a consistent level of parenting.
